Helpful resource guide to help inform you of your rights before leaving the hospital. It includes information on how to appeal your discharge.
This pamphlet is for people who have Medicare, Medi-Cal, or both.
You Have the Right To a Safe Hospital Discharge
DEMAND YOUR RIGHTS
When you are in the hospital you have a right
to get the care you need.
When you return home you also have a right
to have the things you need for your recovery.
If you do not have a home you have the
right to go to a place where you can be cared
for.
Sometimes you have to remind the hospital
that you have these rights. You have the legal
right to medical treatment, a discharge plan,
and a discharge date that works for you.
If you have Medicare or MediCAL
or both, and want to appeal your
discharge, call:
Health Services Advisory Group
(HSAG) at 1-800-841-1602
If you disagree with the care you are receiving
or the discharge plan and you have
private insurance, contact your insurance
company. Every type of insurance has an
appeal process, but these rights vary based
on the type of insurance you have. Talk to
the Hospital Doctor, Case Manager or Patient
Advocate to understand these appeal
rights and to find out how to exercise them.
